Consent to receive SMS

1. Purpose of SMS Messaging

  • You consent to receive unencrypted SMS text messages from Tori Health for the following limited purposes:

  • Appointment reminders

  • Program participation or engagement messages (e.g., check-ins or motivational texts)

  • Referral or care coordination notices (non-diagnostic)

  • General administrative messages (e.g., contact updates)

    Tori Health will never send Protected Health Information (PHI) or sensitive clinical content via SMS.

2. Risks of Using SMS

SMS text messages are not secure or encrypted. Risks associated with receiving SMS include but are not limited to:

  • Unauthorized interception or access by third parties

  • Messages being sent to the wrong number due to errors

  • Forwarding or sharing of messages by recipients

  • Permanent backups that may be stored without user knowledge

  • The potential for spoofed or falsified messages

  • You acknowledge and accept these risks by consenting to SMS communication.

3. Limitations and Appropriate Use

  • SMS must not be used to communicate urgent or emergency issues.

  • Do not use SMS to share sensitive personal or medical information, including but not limited to:

  • Mental health conditions

  • Substance use

  • HIV/AIDS or sexually transmitted infections

  • Suicide risk or crisis-related concerns

  • SMS is not actively monitored by Tori Health staff. A timely response is not guaranteed.

  • If you send a message that requires a reply and do not receive one within two (2) business days, you must follow up by phone or email.

  • Tori Health reserves the right to document relevant administrative SMS in your treatment record when appropriate.

4. Your Responsibilities

By consenting, you agree to:

  • Notify Tori Health promptly if your mobile number changes

  • Take reasonable precautions to protect access to your SMS messages (e.g., password-protect your device)

  • Never rely on SMS as your primary method of contact for health concerns

  • Keep a copy of this form for your records

5. Opt-Out and Revocation

You may opt out of receiving SMS messages from Tori Health at any time by replying STOP to any SMS message received. You may also revoke your consent in writing by contacting us at team@torihealth.com.

This consent remains valid until revoked by you in writing or until terminated by Tori Health.

6. Acknowledgment

By consenting, you acknowledge:

  • You have read and understand the above information

  • You understand the risks of unencrypted SMS messaging

  • You voluntarily consent to receive SMS messages under the described terms
